Living Well Living Longer project was launched in Blaenau Gwent in January 2015 to identify and support those at greatest risk of developing cardiovascular disease. Citizens are offered individual screening and well being plans that include information relating to alcohol, smoking, exercise, and diet.
Kaleidoscope is proud to have been part of the development of this project from its conception and to provide ongoing support, training and supervision to staff across health care disciplines and agencies.
The award was won for “Improving Outcomes Through Learning and Development” and comments were received specifically around excellent partnership working, multi agency support and citizen focused health care.
The award was presented at the Intercontinental Hotel at the O2 in London, and hosted by Sue Perkins.
